| Deficiency Description: 13B - Establishment fails to maintain employee training records. 15B - The framing around the dough mixing station has accumulation of dried food residues. 12A - Cartons of liquid eggs are improperly stored above staged ingredients in the walk-in cooler. 15C - Floor mixer in dough processing area is observed to have a moderate accumulation of dried food residues on the underside of mixing shaft. - Cooling unit fan guards in the walk-in Silver cooler are dust laden. - Oven conveyor is improperly maintained as follows: jagged metal is observed on the outer edges. - French Bread packaging station is improperly constructed as follows: wood levelers are composed of unsealed, soiled wood. - Bread cooling conveyor is not properly maintained as follows: framing has heavy accumulation of dust and oil residues. 02B - Flour bin in the dough mixing area is found to be infested with grain beetles. 25 pounds of effected product was destroyed under signed waiver during inspection. Bin was removed for cleaning. 15A - In-use serving utensils in the retail café are improperly stored in sanitizing solution with undetectable concentration. Discussed and corrected. 14B - 10-20 small live flies are observed on and around the dough mixing units. - 2-3 small dead grain beetles are observed in the waste reservoir beneath the corn meal sifter. - 3-5 dead grain beetles are observed on the floor near the bulk flour tanks. 10C - Floor throughout the production area exhibits damaged surfaces. Missing caulk and grout is noted with standing water within the gaps. - Excessive food residues are observed on the floor in the main walk-in freezer. - Ceiling above dough mixer has a moderate accumulation of rust and flaking paint. - Flaking paint is observed on the ceiling above the overhead proofer. 10A - Exterior loading doors, near the three bay sink, exhibit spaces greater than 1/4 inch at bottoms and/or sides. 05B - The establishment does not have a written food safety plan where required. Hazards requiring a preventive control are present, and not controlled. 04E - The processing area ice machine has extensive build up of dark mold like growth and grime across interior surfaces. Equipment removed from service and properly cleaned during inspection. 02D - Bulk ice cubes in the hopper of ice making unit are contaminated as follows: excessive accumulation of dark mold is noted at the upper interior of the unit. Water droplets are observed to drip from the mold material directly onto the ice below. Approximately 200# of ice was destroyed under signed waiver during inspection. 12B - Ingredient storage bins in walk-in cooler and walk-in freezer are observed to have soiled exteriors. 15F - Storage racks in the walk-in coolers have accumulation of dried food residues. | |||

Discussed with management at time of inspection. 09A - Handwash facility in dough area is observed cluttered with cleaning rags. 16A - Liquid spray bottle in dough prep area not labeled.- Liquid spray bottle in pastry prep area not labeled.- Fly traps improperly hung in retail food conveyance area without catch basin. 11A - Employee medication bottle and drink observed on prep table in rear cookie area.
